Fountain of Faith Free Medical Clinic
Our Policies

1.  Fountain of Faith cannot provide medical care between clinics.  This includes, but is not limited to, medical advice, prescriptions, refills or care.

2.  Fountain of Faith cannot complete forms.  This includes, but is not limited to Disability, Social Security, Physical, FMLA, Work notes.

3.  Fountain of Faith does not prescribe any controlled substances or narcotic medications.

4.  Private offices of volunteer physicians are unable to provide medical care, advice, prescriptions, refills or test results.  Patients seen at the Free Clinic can only contact the Free Clinic.

5.  Though services provided at the Free Clinic are free of charge, medical services outside the Free Clinic may have charges that the patient would be responsible for.  This includes, but is not limited to labs, xrays, medications, etc.

6.  If the patient receives a laboratory or radiological study, the patient will be notified in writing within 2 weeks of that clinic date.
April 3rd Clinic Update

What a wonderful April Clinic!  We provided over 35 office visits to patients from all over North West Ohio.  We were excited to assist patients with meds as well as labs and Xrays. We had several patients sign up for our smoking cessation program!

Our additional exam rooms are wonderful!                                                                             These not only reduce wait times but also enable us to see more patients.  Thank you Community Memorial Hospital for donating the space, supplies and IT support for this amazing expansion!
